Once we have registered a death, you must request corrections at Correct a death certificate.
We assess all corrections before making them.
You can choose to:
- Request a correction yourself, or
- Guide families in this process.
There is no charge if you return the original death certificate to BDM.
Request a correction on behalf of the family
Create an account
Before requesting your first correction, you must create a new account on our public-facing system.
You can’t request corrections from your funeral director account.
To create an account:
- Go to Changes and corrections > Correct a death certificate
- Click on Apply now, then Create a new account
- Use the same email address and password you used to create your service partner account
- Log into this account for any corrections you request on behalf of families.
1 Get the informant to sign a correction consent form
- Ask the informant to complete and sign a Death certificate correction consent form
- Scan or take a photo of the signed form ready for attaching to your correction request.
2 Fill in a request for correction
- Log into your account at Changes and corrections > Correct a death certificate > Apply now(opens in a new window)
- On the Applicant details page:
- For ‘What is your relationship to the subject’: Select ‘Other’, then enter ‘Funeral director’
- On the Registration details page:
- Enter the death registration number
- On the Correction page:
- Enter details of the correction
- Select ‘Add documents’ to upload:
- A copy of the signed correction consent form
- For Requirement type, select ‘Proof of Error’
- For Document name, choose ‘Other evidence to substantiate’
- Any evidence required for the correction (see Correct a death certificate for types of evidence)
- A copy of the signed correction consent form
3 Order a replacement certificate, or return the original
We ask you to return the original certificate(s) before we can issue a new, corrected version.
If you return the original certificate(s)
Post the original certificate to our mailing address. Please include your order number.
Once we’ve corrected the record, we’ll send you a new certificate(s) free of charge.
If you can't return the original
Once we've corrected the record, you'll need to apply for a new death certificate. There is a fee for the new certificate.
We recommend that you don’t order a new certificate until we’ve corrected the record.
Please note that the certificate turnaround time is separate to the correction turnaround time.

- How to provide evidence of the correction
- Proving identity
- Processing and delivery
