Child's consent to change of parent's details

Your parent is applying to change their details on your birth certificate.

You may fill in this form if:

• You are 12 years or older (but under 18)
• You want your birth certificate to show your parent’s current details.

You must sign this form in front of a witness who is 18 years or older and not a parent or guardian.

If you are 18 or older then you are responsible for making the application to change your parent’s details.
